La glomerulonefritis colapsante es una enfermedad poco frecuente que puede estar asociada a distintas causas, una de ellas son las enfermedades autoinmunes y dentro de estas el lupus eritematoso sistémico (LES). De manera frecuente se presenta con un cuadro de severas alteraciones renales que tienden a progresar la enfermedad renal terminal, con escasa respuesta a los tratamientos. Se presenta un caso de glomerulonefritis colapsante asociado a lupus eritematoso sistémico que tuvo una respuesta completa al tratamiento de inducción con la combinación de glucocorticoides, antimaláricos y mofetil micofenolato iniciado precozmente por el diagnóstico temprano realizado por biopsia renal.

Collapsing glomerulonefritis is a slightly frequent disease that can be associated to different causes. Autoimmune diseases are part of those, and inside these, the systemic lupus erythematosus (SLE). It frequently appears with manifestations of severe renal alterations that tend to develop the renal terminal disease, with scanty response to the treatments. It is presented a case of collapsing glomerulonefritis associated to systemic lupus erythematosus that had a complete response to the treatment of induction with the combination of glucocorticoids, antimalarials and mycophenolate mofetil used prematurely after the early diagnosis performed by renal biopsy.

Amyloidosis as a cause of heart failure

El término amiloidosis cardiaca hace referencia a la afección del corazón como consecuencia del depósito de amiloide en el tejido cardiaco, ya sea en el contexto de una afección sistémica o de una forma localizada. Se presenta un caso donde se analiza la fisiopatología y el diagnóstico de esta enfermedad. La miocardiopatía por amiloidosis es una enfermedad inflamatoria infiltrativa que con frecuencia no es sospechada. El ecocardiograma no siempre permite identificar el clásico patrón infiltrativo restrictivo.

The term cardiac amyloidosis refers to a heart affection which is a consequence of the accumulation of amyloid in the heart tissue that can appear in the context of a systemic affection or in a localized form. A case is presented and it is analyzed the physiopathology and the diagnosis of this disease. Miocardiopathy by amyloidosis is an infiltrative inflammatory disease that is not frequently suspected. Echocardiogram not always allows identifying the classic restrictive infiltrative pattern.

Individual sensitivity as a health determinant of persons

Se ratifica la importancia primordial de las determinantes sociales para explicar la salud, tanto de las poblaciones como de los individuos, pero especialmente en el caso de estos últimos, los factores biológicos no se pueden dejar de tener en cuenta, dada la heterogeneidad y singularidad de las personas. La susceptibilidad individual a padecer las enfermedades es bien conocida por los médicos que atienden pacientes desde antaño. Se plantea la necesidad actual de revalorar el peso de los factores biogenéticos en las determinantes de la salud de las personas, a la luz de los nuevos logros en el campo de la Biología y, concretamente en la genética, así como por la biotecnología, en el "diagnóstico molecular". Se alerta que las nuevas técnicas que se van disponiendo deben implementarse de manera apropiada, acorde con la ética requerida y no deben inducir a que se abandone el método clínico, incluida la habilidad de recoger una buena historia clínica y una evaluación integral en cada paciente, las que siempre serán importantes para identificar las personas susceptibles que concurren a cualquier escenario asistencial.

It is ratified the paramount importance of the social determinants to explain the health of the populations and of the individuals. Especially in the case of individuals, the biological factors must be taken into account, given the heterogeneity and singularity of the persons. Individual sensitivity to suffering some diseases is well-known for the doctors that have attended patients from a long time. It appears the current need to revaluate the influence of biogenetic factors in the health´s determinants of the persons, in the light of new achievements in the field of the Biology and more specifically in Genetics; as well as for the Biotechnology, in the "molecular diagnosis ". There is alerted that the new technologies that has been emerging must be implemented in an appropriate way, according to the needed ethics and they must not induce to abandon the clinical method, including the skills of collecting good clinical records and performing a comprehensive evaluation in every patient, which will be always important to identify sensitive persons who attend to any healthcare service.
