Updating on the diagnosis of human leptospirosis

La leptospirosis humana es una enfermedad zoonótica de distribución mundial, con frecuencia subdiagnosticada por presentar un amplio espectro de manifestaciones clínicas. El objetivo es presentar una actualización sobre el diagnóstico de la leptospirosis humana. Se consultó la bibliografía disponible sobre el tema en las bases de datos de Scielo, HINARI, Pubmed-Medline y diferentes textos y artículos en los que se profundizaba en el diagnóstico. Las técnicas microbiológicas son la base de la confirmación de la enfermedad. La observación por microscopía de campo oscuro es poco sensible, ya que las leptospiras se pueden confundir con filamentos proteicos u otros artefactos. El aislamiento del agente etiológico constituye la prueba de oro, aunque ofrece un resultado retrospectivo. La reacción en cadena de la polimerasa es un método útil para el diagnóstico rápido de la infección. El diagnóstico serológico cobra vital importancia en esta entidad, pues supera en rapidez, sencillez y bajo costo al cultivo. La microaglutinación con antígenos vivos es la técnica de referencia. Las pruebas rápidas basadas en la inmunocromatografía de flujo lateral son una variante muy útil, ya que ofrecen el resultado entre 5 y 30 minutos.

The human leptospirosis is zoonotic disease with worldwide distribution, frequently present a difficult diagnosis because have a wide spectrum of clinical manifestations. The objective of this work is to present an upgrade on the diagnosis of the human leptospirosis; for it was consulted it the available bibliography on the topic in the databases of Scielo, HINARI, Pubmed-Medline and different texts and articles of the last five years to deepen in the diagnosis. The microbiologic diagnostic is the base of the confirmation of the illness; it depends on the taking of sample in the appropriate moment and the correct indication of the complementary one according to the clinical phase. The observation for microscopy of dark field is not very sensitive since the leptospiras can made a mistake with poetics filaments or other devices. The etiologic agent's isolation constitutes the gold standard test; although offers a retrospective result. The polymerase chain reaction is a method used for the quick diagnosis of the infection, the quantitative variant in real time shows substantial advantages, because it allows the immediate reading and avoids the electrophoresis step, these techniques are not available for its high cost. The serologic diagnostic charges vital importance in this entity, it overcomes in speed, simplicity and low cost to the cultivation; the microagglutinación with a live antigen is considered the reference technique. The detection of IgM for enzyme linked immunobsorbent assay has been broadly used. The rapid tests based on the lateral flow immunochromatography, are a very useful variant, since they offer the result between 5 and 30 minutes.

Neurolytic blockof recurrent neuromas guided by ultrasound

Los neuromas de muñón son causa de dolor intenso de difícil control. El bloqueo neurolítico es una alternativa en el tratamiento, y el uso de la ultrasonografía se impone para garantizar la efectividad del proceder. En este reporte de caso se describe la realización de la neurolisis con alcohol absoluto de varios neuromas recidivantes en un muñón de miembro superior, guiados por ultrasonido, en un paciente con dolor crónico que no resuelve con tratamiento médico.

Stump-neuromas cause difficult deep pain control. Neurolytic block is an alternative treatment, and ultrasound guarantee the certainty of the procedure. This case report describes the realization of neurolysis with total alcohol of various stump-neuromas, guided by ultrasound, in a patient with chronic pain that don´t solve with medic treatment.

Dischromya in Upper righ central incisive

Paciente masculino de 52 años de edad, tez blanca, residente en una comunidad urbana en La Habana, sin antecedentes patológicos referidos; que acude a consulta estomatológica preocupado por su estética; al examen bucal se observa cambio de color en el diente 11 (Incisivo central superior derecho) hacia carmelita oscuro que evidencia una discromía. A la imagen de una radiografía periapical se observa previo tratamiento pulpo radicular (TPR), por lo que se decide comenzar un tratamiento de recromia; en 5 consultas a intervalos con posterior restauración del diente en cuestión. Después del tratamiento se observó recuperación del color normal del diente, mejoría en la estética y gran satisfacción nuestra y del paciente.

A 52 years old male patient of white race, resident in an urban community of Habana City, he does not suffer from any disease; he was worried because his aesthetic, the oral examination shows a different colour on tooth 11 (upper righ central incisive) to dark brown that confirm a diagnosis of dischromya. The periapical x-ray disclosed a previous root canal treatment, according to the signs the treatment to recover from the disease is a rechromya, in 5 appointment with a dental restoration the patient presents good evolution and report a better appearance and a great satisfaction from ours and the patient.

Usefulness of Doppler echography for the diagnosis of traumatic carotid thrombosis

Los traumatismos del sistema arterial carotideo son raros y tienen alta mortalidad. Se presenta un caso de trombosis de la arteria carótida interna derecha por una herida por arma de fuego a nivel del cuello con el objetivo de evidenciar en la práctica médica la utilidad de la ecografía Doppler para el diagnóstico temprano de estas lesiones, a pesar de reportarse en la literatura la angiografía, la resonancia magnética nuclear y la angiorresonancia como estudios de elección. Con este caso se pudo afirmar que la trombosis carotidea traumática tiene una incidencia baja, pero no descartable en los traumas de la región cervical. Por esta causa, son frecuentes las complicaciones neurológicas dadas por la oclusión arterial y la ecografía Doppler resultó muy útil en el diagnóstico temprano del paciente.

Carotid artery system traumas are rare and show high mortality rates. The case of right internal carotid artery thrombosis caused by a gunshot injury in the neck was presented in this report to show evidence of the usefulness of Doppler echography in the medical practice for the early diagnosis of these lesions in spite of the fact that literature highlights angiography, nuclear magnetic resonance and angioresonance as the imaging studies of choice. This case confirmed that carotid thrombosis due to trauma has low incidence but it can not be ignored in the cervical región traumas. For these reasons, the neurological complications stemming from artery occlusion are frequent and Doppler echography turned to be very useful for the early diagnosis of this illness in a patient.

The present work shows an informatic product that helps to teach and study the method for the conformation of the clinical history in integral and general dentistry.An exhaustive compilation is presented about the main aspects needed for studying the isuue mentioned above.The structure oh the digital document allow a quick and easy acces to the content and make it usefull both as a basic text for junior students and as a consulting material for graduated.

Cajal and Marti: paradigmatic coincidences and similarities

En Cajal y Martí se aprecian semejanzas en sus aportes e ideales. Ambos coincidieron en su juventud en el periodo de 1871 a 1873 como estudiantes en la Universidad de Zaragoza, institución docente en la que recibieron sus títulos universitarios. En 1873, Cajal se graduó de Licenciado en Medicina y Martí, un año después, de Licenciado en Derecho Civil y en Filosofía y Letras. El objetivo del presente trabajo es realizar un análisis comparativo para identificar coincidencias y similitudes que permitan destacar las virtudes y los valores compartidos, patrióticos y humanos, en estas dos personalidades de proyección universal, verdaderos arquetipos de la noble expresión de las mejores cualidades de dos pueblos hermanados por la historia.

There are similarities in the contributions and ideals of Cajal and José Martí. Both lived in the same period of time of 1871 to 1873 as students in Universidad de Zaragoza from which they graduated. In 1873, Cajal graduated as Bachelor of Medicine whereas Marti majored as Bachelor of Civil Law and Philosophy and Literature the following year. The objective of the present paper was to make a comparative analysis that identified the coincidences and similarities between both men, which would allow highlighting shared virtues and values, either patriotic or human, of these two universal figures who are real architypes of the kind expression of the best qualities of two peoples twinned by history.
