Resumen

ABREU MERIDA, Magalys; SANCHEZ MARTINEZ, Nitza  y  PILOTO MOREJO, Manuel. Postsurgical climacteric. Rev Cubana Obstet Ginecol [online]. 2001, vol.27, n.2, pp. 96-105. ISSN 1561-3062.

A retrospective, cross-sectional and descriptive study was conducted at “Justo Legón Padilla” and “Ciro Redondo” hospitals, in Pinar del Río, from October, 1988, to December, 1999, in order to contribute to the knowledge of postsurgical climacteric. The study group was composed of those female patients who were surgically castrated at any hospital of the territory or outside it (n = 84) and that were seen at the provincial consulting room of Gynecology. A survey with variables such as age at the moment of castration, precastration and postcastration history, parity, surgery performed, symptoms, signs and their time of appearance, causes of the operation, histological confirmation and treatments used, was done. The main results were as follows: mean age of 35.27 (SD = 7.39) at castration; increase of heart diseases, arterial hypertension and arthropathies after castration; and mean parity of 1.44 (SD = 0.82). Most of the symptoms appeared less than a year after the operation and the most frequent were: flush or heat (95.24 %), depression (5.95 %), reduction or lost of sexual desire (70.24 %) and articular and muscular pain (64.29 %). The main causes of castration were: uterine fibroma (33.33 %), uterine cancer (19.05 %) and uterine hemorrhage (13.10 %). An improvement was observed as a response to hormone replacement therapy with all the combinations used. It was concluded that there is an exaggerated abuse of castration without justified causes, such as cancer and genital sepsis. It was recommended to maintain the ovaries in any woman under 50 operated on and in those women over 50, according to the surgeon’s criterium, and to discuss these preoccupying results with the General Surgery Professorship.
