This paper presents a 38 years-old patient who suffered an blunt fracture in his right tibia. He was surgically treated with external fixators and progressed to chronic osteomyelitis. Necrectomy and sequestrectomy were applied in several occasions to solve the bone lesion but the sequela was a skin defect located in the anterointernal side of the medial third of the leg, with exposed bone tissue. Then, a flap with proximal vascularization of the soleus muscle was used as a cover together with a partial skin graft. The patient recovery was satisfactory.
