The subarachnoidal hypobaric anesthesia is a regional method for anorectal surgery. In order to describe the effects of bupivacaine 0.1 % with this procedure, 50 patients scheduled in "Dr. Carlos J. Finlay" Military Central Hospital were studied. The following variables were studied: anesthesic level, motor blockage degree, quality, duration of analgesia and complications. A low anesthesic level (T12) was attained on 45 of the patients (90 %), with preservation of the motor activity in 46 of them (92.0 %). The quality of surgical analgesia was good in all the patients, and the average time of postoperative analgesia was 331.2 min. Complications were reported in 7 cases (14 %), and urinary retention was the most common. Hypobaric anesthesia with bupivacine 0.1 % is an efficient method for anesthesia in anorectal surgery.
