It is presented the case of a masculine, 32 years old patient, who was taken to the surgery room with the diagnosis of severe cranial trauma with complicated depressed fracture. He developed a massive atelectasis of the left lung for a mucous plug in the postoperatory period, proving some clinical manifestations of it during transoperatory which disappear when the patient was moved from the stretcher to the operation table, apparently for mobilizing the mucous plug. At postoperatory, it was necessary a bronchoscopia with bronchial lavage and extraction of secretions. It was reviewed the most frequent causes that bring about this affectioin in the perioperatory.
