Background: avascular necrosis of the femoral head is an infrequent lesion, especially when it is located in other areas of the proximal femur like the femoral neck. Objective: to present the case of a patient with atypical location of avascular necrosis in the proximal femur. Clinical case: a forty-nine-year-old white female patient with a personal medical record who six months ago started presenting pain in the right hip. The intensity and duration of the pain gradually increased. It also got worse at walking and standing for long periods of time, associated with limp. A simple radiographic examination showed an osteolytic image of well-defined edges in the anteroposterior side of the right hip. To check the presence of this image, a CT scan was performed. A sliding screw fixation was placed to keep the normal anatomical structure and avoid a pathological fracture. Conclusion: the location of avascular necrosis out of the femoral head is extremely infrequent. In these cases other surgical procedures, like resection and fixation with nails and screws, are needed particularly when the location is at the level of the femoral neck and there are risks of a pathological fracture.
