Introduction: The prone ventilation is a pulmonary protection measure used from 30 years ago in treatment of respiratory distress syndrome in adult patient and the acute pulmonary damage. Objective: To analyze the hemodynamic and hemogasometric effects from application of prone ventilation in patients presenting with acute pulmonary damage as a measure of pulmonary protection. Methods: The selected patients were placed in prone position for 4 hours of mechanic ventilation. In three times related to this position change it was possible to collect data that were analyzed with the Student t method with a 95% CI and a significance level for all the positions, p 0.05. Results: The arterial tension, heart rate and central venous pressure obtained in dorsal-decubitus position were not modified regarding its values of prone-decubitus and two hours after be placed in supine position. The oxygen arterial pressure, the oxygen hemoglobin saturation, oxygen alveolar-arterial difference and the Kirby rates of efficiency and prognosis, improved in the prone position and remain values higher two hours after to be changed to supine decubitus position. It was neither necessary the hemodynamic support nor the muscular relaxation to tolerance method. In four patients sedation was necessary. The increase of the endotracheal discharges drainage was a frequent technical problem. Conclusions: The prone position ventilation contributed to improve the blood oxygenation in patients presenting with a severe hypoxemia.
