Insulinoma is a rare tumor of beta cells of islets of Langerhans. It is characterized by uncontrolled and excessive secretion of insulin, resulting in hypoglycemia after prolonged fasting or exercise. As part of the surgery to treat this condition, the anesthetic management poses a challenge for the anesthesiologist due to the difficulties in maintaining normal blood glucose levels. We present the case of an insulinoma patient who underwent capillary blood glucose monitoring during surgery every ten minutes. This allowed keeping blood glucose levels within normal limits. The use of balanced general anaesthesia with endotracheal intubation led to a successful surgical procedure, which was guided by an intraoperative ultrasound.
