Epidermoid carcinoma is a malignant neoplasm of epidermal cells, reason for it only appears on the skin and mucous membranes; between 75-90% in head and neck. It is the second most frequent cutaneous neoplasm among those of the non-melanoma type, with a predominance of 2.1: 1 in the male-female ratio and the cancer that most often causes metastasis. This case presentation aims at describing the surgical technique of Esser’s bilobed flap for resection of an epidermoid carcinoma located in the outer edge of the eye, a technique not indicated for this region of the face, but with which a satisfactory result was obtained in the treatment of this case, A 75-year-old white-skinned patient who attended the Maxillofacial Surgery Service of the General Hospital of Cienfuegos, referred by the Dermatology Service, due to the presence of an ulcerated lesion of approximately 5 cm. in diameter located in the mentioned region. It is concluded performing a bilobed flap for resection of the lesion was the ideal method to obtain favorable postoperative results, despite not being indicated for this location on the face.
