Smoking is considered the most important modifiable risk factor due to its local and systemic effects on the periodontium. We conducted an updated literature review in order to describe the pathophysiological effects of smoking as a risk factor for periodontal disease based on the molecular events involved in its etiopathogenesis. Smoking contributes to the transition to periodontitis since it influences the formation of bacterial plaque, and the inflammatory response as a result of the harmful effects of tobacco components, especially their vasoconstrictive action and osteoclastic activity. It is associated with a variety of deleterious changes in the oral cavity, affects all its components, alters its microenvironment, and in turn, predisposes it to various conditions.
