Intestinal occlusion is a medical emergency whose treatment must be imposed, as soon as an accurate diagnosis is established, to avoid complications. Objective: To describe the distinctive clinical and surgical aspects of a patient with intestinal obstruction due to renal cyst at Saturnino Lora Provincial Hospital in 2020. Clinical case: An 89-year-old male patient who visits the doctor because of stool and gas retention; given its clinical manifestations and the results of complementary tests, he is diagnosed with intestinal occlusion due to a colon tumor. During the intraoperative period, a compressive renal cyst with a hepatocolic angle was found, measuring 18 centimeters in diameter. Marsupialization of the right renal cyst is performed. Conclusions: The giant cyst determined mechanical intestinal occlusion with typical manifestations, that is, stool and gas retention, a globular and distended abdomen with generalized bloating. Marsupialization demonstrated efficacy by keeping the patient in an adequate safety profile.
