We presented the case of a 32 years-old black male patient that was seen at the doctor's because he had hemoptoic expectoration that occasionally contains microlites. Chest X-ray exam showed intraparenchimatous calcification in the lungs. Transbronchial biopsy was performed and the final diagnosis was alveolar pulmonary microlithiasis. The case was discussed and a literature review was made
