An updated review of ischemic preconditioning and of the most accepted pathogenic hypothesis of cardiac protection exerted by the late stage or second window of ischemic preconditioning was made. The most recent data generated by the intense research in this field show that the generation of this defensive phenotype of the cardiomyocytes is a polygenic phenomenon that requires the simultaneous activation of multiple stress genes. The hypothesis of nitric oxide as a dual element, initiator and mediator, has allowed to give a coherent explanation about the interconnection of the main molecular mediators involved in this cardiac protection. The identification of the molecular bases of this phenomenon will undoubtedly provide the appropiate conceptual framework for the development of new therapeutical strategies going from the use of drugs or process mimicking the second window of the ischemic preconditioning to the application of genetic techniques making possible to maintain the myocardium in a sustained defensive state.
