A review was made to explain how the diverse cellular processes occurring in the severely damaged liver tissue and in the mesenteric circulation influence on the different systemic alterations appearing in acute liver failure, and the close relation existing among them. The acute liver failure is a clinical syndrome resulting from a mass liver necrosis generating progressive dysfunction of the organ with the presence of deep alterations in its detoxicating, metabolic, sinthesizing and excreting functions.The hypothesis of critical mass, as well as that on endotoxins-cytokines and vasoactive substances explain somehow the different molecular changes supporting the physiopathological bases of this complex entity.
