Dismay and syncope are among the most frequent medical phenomena and practically all the adults have experienced some presyncopal symptoms, a totally developed syncope, or they have observed it in other persons. The vasovagal syncope is the commonest one. The incidence of the syncope of unknown origin in some reports is up to 47 %. The loss of consciousness may be caused by many disorders, most of them of bening origin, such as the vasopressor reaction, which is the most common and non-recurrent; however, in a small number of cases, the syncope may be produced by a disorder having danger for life as the ventricular tachyrhythmia and be premonitory of sudden death. The most frequent mechanism of syncope is the bradycardiac reflex. The study of syncope is a complex problem requiring a clinical guide to rationalize it. There is no evidence of the diagnostic usefulness of the neuroimage tests, electroencephalogram or vascular studies in the patients with syncope when there are no signs of neurological focus. The tilt-table test is the diagnostic test with the highest income-yielding capacity in the study of patients with syncopes or unclear cause. The cases with syncope should be referred to the neurologist only when there are neurological signs of focal dysfunction, epileptic seizures are suspected, or when the cardiologist's evaluation, including the tilt-table study, is negative.
