The definition of ischemic or haemorrhagic etiology of an ictus is based on clinical parameters and on the cranium computerized axial tomography (CAT). This latter has led to a reinterpretation of the clinical signs, with etiological and topographic diagnostic ends, and of the complications. However, it is not always available in the hospitals. This situation has motivated the creation of clinical scoring systems that pretend to improve the validity of the etiological diagnosis on purely clinical bases. In order to assess the validity of the 2 clinical scoring systems, as well as the diagnostic criterion of the doctors on duty in the etiological diagnosis of ictus, the sensitivity, specificity and positive predictive value were calculated. The diagnostic accuracy of the etiology of 112 ictuses was attained according to the result of the CAT, or of necropsy. It was concluded that the validity of the clinical judgement of the doctors that were on duty calculated on the basis of the informational indexes used was higher than that of the 2 clinical scoring systems.
