Laparoscopic cholecystectomy has become the treatment of choice in symptomatic uncomplicated cholelithiasis; however, this procedure has been controversial for the management of acute cholecystitis because it is considered to be associated with more complications. With the aim to describe our experience with laparoscopic cholecystectomy in acute cholecystitis and assess its feasibility and safety, a descriptive study of the 284 laparoscopic cholecystectomies undertaken for acute cholecystitis between July 1st., 1996 and June 30th., 2002, in the Minimal Access Surgery Center of the "Manuel Ascunce Domenech" Provincial Teaching Hospital of Camagüey was performed. 66,9% of patients were female. The mean age was 54,2 years. Time of onset of symptoms was 72 hours or less in 251 cases (88,4%). Mean surgical time was 51,1 minutes. Six patients were converted to open cholecystectomy (2,1%). There were 10 complications, which represented a 3,5% complications rate. There was no mortality. 93% of patients had a post-operative stay less than 24 hours.We conclude that laparoscopic cholecystectomy in acute cholecystitis is a safe and feasible technique, with low morbidity, and sholud be the procedure of choice in these patients within 72 hours of onset of symptoms.
