A descriptive study was performed to evaluate the utilization of Doppler transcraneal ultrasonography in the diagnosis of the brain death from January 2002 to May 2003 at Manuel Ascunce Domenech Provincial Hospital. The universe was composed of 45 patients whose data were processed by Microstat program. Osseous temporal and orbital windows were used in all patients. Nine subjects presented diastole-systole separation pattern all within the first two hours of the brain death diagnosis. Reverberant and spicular flow was show in 34 sick patients only seven of them in the first two hours. In two individuals (both women) it was not detected flow through the temporal window but in the orbitary it was detected. With the use of the transorbitary window and the extranscranial internal carotid artery, the percentage of positive results for the brain death was elevated to 100 %.
