Background: patients with oral cancer present a poor survival of five years. Many of the lesions are developed on leukoplakia and erythroplasia therefore it is necessary to achieve advances in the diagnosis of oral precancer. Objective: to describe recent advances in the precocious diagnosis of oral precancer that may allow us to value its progression and recurrence. Method: the most recent articles that were reviewed on computerized databases as: MEDLINE, CANCERLIT, EMBASE dealt with: injuries and cancerous states, molecular changes in carcinogenesis, clinical examination and diagnostic techniques. Results: molecular markers will become essentials for diagnosis and patient´s management with oral cancer. It allows for precocious diagnosis and the staging of tissular changes before morphological changes in the cells happen. Contribute therefore to achieve a better survival and less aggressive treatments. Conclusions: it is concluded that clinical examination, biopsy and the employment of molecular markers are fundamental in diagnosis of cancerous and malignant lesions of the oral complex. Also, advances achieved at molecular level have improved our understanding in the pathogenesis of oral cancer.
