Background: the production of hormones is governed by certain biological and circadian rhythms. Prednisone is the treatment of choice for minimal damage by nephrotic syndrome. If the biological rhythm and treatment is coordinated, it is possible to modify its effectiveness. Objective: to compare the effectiveness of a single dose coordinated with circadian rhythm and fractional dose. Method: a single dose was administered to a group of patients and fractional dose to another one. To the results, was applied the Fisher's exact probability test and Student´s t test. A significance level of α=0.05 was selected. Results: with the single dose, complications were less frequent; the time of disappearance of proteinuria was shortened and decreased the cost of hospitalization. Conclusions: single dose of prednisone had advantages compared with fractional doses.
