Thyroid anaplastic carcinoma (TAC) is one of the most malignant neoplasias affecting the human beign, with a survival of less than 6 months after diagnosis. The aim of this paper is to analyze its form of presentation, combined modality treatment and the evolution of those patients presenting with TAC at the National Institute of Oncology and Radiobiology. We studied 25 patients with TAC from December, 1967, to December, 1997, at the Splanchnic Surgey Service of this center, where these patients received comprehensive attention (diagnosis, treatment, follow-up and death). The positive diagnosis was made by fine needle aspiration biopsy citology. The treatment consisted of neoadjuvant radiotherapy and drug therapy plus surgery. Average age of patients was 64 years (range 55-70), the male/female ratio was 1:1.5 and disease frequency was 4.6 %. The most common form of presentation was a fast enlargement of the gland associated with cervical pain, dysphonia, dyspnea and dysphagia. Invasion to adjacent structures, distant metastases and regional cervico-ganglionic metastases were present in 68 %, 32 % and 20 % of the cases, respectively, at the moment of diagnosis. Complete resection was achieved in 7 patients and partial in 10. There was no surgical mortality. The most frequent histologic type was the fusiform cell TAC. The average survival of the patients that underwent complete resection of the tumor was 24 months
