This is the case of a patient aged 64 seen in the emergency room due to a acute thorax pain located in right hemithorax and intense dyspnea. The persistence of pneumothorax without to achieve a pulmonary re-expansion with a pleurotomy and thoracic drainage be necessary a thoracotomy. A ruptured giant emphysematous bulla was found thus the superior and middle bilobectomy. The histological diagnosis was a bronchoalveolar carcinoma. Coincidence of a lung cancer in a emphysematous bulla isn't exceptional and the persistence of pneumothorax as initial manifestation of lung cancer is not frequent.
