The aortopulmonary window (APW) is an uncommon congenital heart malformation that consists in the communication between the pulmonary artery and the ascending aorta above, respecting both sigmoid valves. From 1987 to 2002, 14 patients out of the 16 with this diagnosis underwent surgery at the Heart Center of "William Soler" Pediatric Teaching Hospital. Patients were classified according to the type of defect following the classification of the Nomenclature Committee of the Society of Cardiothoracic Surgeons. The frequency of presentation is described by age groups, associated anomalies, surgical technique used, morbidity and mortality. 4 patients over 1 were operated on. The most common associated anomalies were the interventricular communication and the persistence of the arterial duct. 50 % of the patients underwent closure of the communication by longitudinal aortotomy and synthetic patch. The closure by pulmonary arteriotomy required a shorter time of aortic clamping. Mortality was 21.4 %. Pulmonary hypertension and the associated anomaly were related to the morbidity and mortality observed. The surgical treatment of the aortopulmoanry window may be successfully performed when it is diagnosed during the first months of life and by transpulmonary route.
