From September 1991 to December 1993, 253 women were operated by the Classical Intrafascial SEMM (Serrated Edged Macro Morcellate) Hysterectomy; this CISH technique by pelvioscopy was indicated to 152 patients, and to 101 patients the indication was of the CISH technique by laparotomy. Uterine leiomyomatosis with menstrual abnormalities and compressive symptoms was present in most cases (61 %), and was the main indication for hysterectomy. The uterine size occupied and important place in determining which patients were going to be operated by pelvioscopy or by laparotomy. The histological findings were correlated with the indications for surgery. Leimyomas and leiomyomatosis with adenomyosis were the most frequent findings. Uterine neck displasia is not a contraindication for the CISH technique, but emphasis is made on the importance of an adequate preoperative assessment. Histological analysis reveal that the scaly-columnar transformation zone was totally removed in all the cases, and that the cervical glands were eradicated in 92 % of the cases. CISH technique is a conservative hysterectomy, which can minimize the uterine neck cancer incidence.
