Syphilis is an infectious disease caused by the spirochete Treponema pallidum, subsp. pallidum; it is usually acquired by sexual contact or by vertical transmission during pregnancy (from mother to child). Syphilis stands out among infectious diseases due to its wide variety of clinical manifestations, and it has been called the great simulator. We present a black patient with multiple annular lesions on the external genitalia, the pubis, and the upper internal thigh region. Non-treponemal serological tests were performed and gave positive results. An early acquired symptomatic cutaneous syphilis (annular syphilis) was diagnosed.
