Introduction: the advanced gastric cancer is the second reason of death due to malignant cancer in the world. There it is a surge of this disease in the last five years in the province of Matanzas. Objective: to determine the clinical-epidemiologic behavior of advanced gastric cancer diagnosed by video-endoscopy in the Department of Gastroenterology of the Teaching Hospital “Comandante Faustino Pérez Hernández”, of Matanzas, in the period from January 2014 to January 2016. Materials and Methods: an observational, descriptive and prospective study of the department of Gastroenterology of the Teaching Hospital “Comandante Faustino Pérez Hernández”, of Matanzas, was carried in the period from January 2014 to January 2016. The universe were 28 patients who had advanced gastric cancer according to the endoscopic and histological diagnosis. Outcomes: the most affected age group was the one of the patients aged 60 years and more (67,9 %).The male sex predominated, with 57,1 %. The risk factors with higher incidence were an inadequate diet (65 %) and smoking (42,9 %). The most relevant clinical manifestations were weight loss, asthenia and heartburn. The predominating histological variety was the intestinal type adenocarcinoma and the gastric antrum was the most frequent location. Most of the patients were positive to the urease test for helicobacter pylori (60,7 %). Conclusions: the infection by Helicobacter pylori is one of the main causes of gastric cancer. The precocious diagnosis and treatment of the infection will favor the reduction of its incidence.
