Purple discoloration of the urinary catheter bag is very unusual and striking for assistants, nurses and physicians, leading to many questions and possible explanations. This phenomenon is very alarming, but benign. It mostly occurs in elderly, bedridden, long-term catheterized patients with recurrent urinary tract infection. The second case of purple urine bag syndrome treated in the Gerontology and Geriatrics Department of the Dr. Gustavo Aldereguía Lima Hospital is presented in order to enlighten young professionals about this very unusual situation.
