The experience obtained with the implementation of a cost system by area of responsibility and its use as a management tool in 54 family physician's offices grouped in 3 Basic Working Groups (WBGs) at "Luis Pasteur" Polyclinic, in Havana City, is presented. An open trend toward the decrease of the costs by consultation, both in family physician's offices and in WBGs is observed. 20 of them as well as 2 of the 3 WBGs reached a unitary cost under $2.00. The cost by consultation of the family physician's office (1.93) is lower than that of the polyclinic (3.16), due to the existance of areas of specialized consultation and interconsultation, where the utilization of the medical resource is not optimal. The data obtained allow to create historical series in conjunction with the commented experiences of the work carried out in favor of the reduction of costs and to improve positively the medical and social efficiency. In a near future it will be possible that the consultations of the Family Physician Plan be lower than $2.00, which convincingly shows its economic viabilty
