Renal disease is uncommon in systemic sclerosis but, at the same time, it is a severe complication occurring more frequently in a diffuse clinical form. The behavior of a female patient with diffuse systemic sclerosis that developed a complicated refractory arterial hypertension, anemia and renal failure is described. Renal biopsy was made with a histopathological description consistent with an ischemic kidney disease associated with this disease which had a poor prognosis despite the treatment with angiotensin converting enzyme inhibitors (ACEI).
