The case of a 60-year-old female patient that seeked medical assistance due to intense vulvar pruritus is presented. She did not resolve with the treatment and she began to notice changes of the color of the skin and flattening of the cutaneous folds of labia majora with ulcerated and fetid papules. According to the result of the biopsy, she had lichen sclerosis et atrophicus. Treatment with conjugate estrogens and antihistaminics was started, but little improvement was observed. It was finally decided to perform simple vulvectomy with resolution of the symptoms.
