A 73-year-old female patient with uncontrolled type 2 diabetes mellitus and urinary lithiasis, comes in the emergency department of Hospital Lenin from Holguín, Cuba. She had intense right lumbar abdominal pain, dysuria and nausea. Ultrasound showed renoureteral dilation, and plain radiography of the urinary tract reported image of right ureteral lithiasis. The urine culture proved the presence of Escherichia coli sensitive to ciprofloxacin, which was medicated intravenously. Due to unfavourable evolution a CT scan was performed and it showed gaseous pattern in renal cavities, pyeloureteral dilation and impacted lithiasis in lumbar ureter. Extractive lithiasis surgery was done and a JJ ureteral catheter was left in place. The evolution was favorable. She was discharged from the clinic after a 10 months follow-up, asymptomatic, with a diagnosis of emphysematous pyelonephritis.
