The case of a patient with giant simple hepatic cyst, a rare affection that is usually asymptomatic and results from an imaging finding, was described. When the cyst reaches a size over 5 cm,  it starts to produce clinical manifestations, as in the case under study, that was successfully treated by minimally invasive surgery.  A simple hepatic cyst was anatomopathologically confirmed.
