INTRODUCTION. Circumferencial mucosectomy with autosuture stapler described by Longo as a technique for treating grade III and IV hemorrhoids have extended in the last years. The objective of the present study was to analyze the preliminary results obtained with this technique. METHODS. 50 patients with grade III-IV symptomatic hemorrhoidal symptomatology have been operated on during 16 months, and the surgical time and postoperative complications have been analyzed. A visual analogic scale was used to measure pain. RESULTS. The mean surgical time was 22 min and the average postoperative stay was 2-3 days. Among the postoperative complications, 2 patients (4 %) were reoperated on because of bleeding, and 4 (8 %) needed vesical catheterization due to urinary retention. The pain intensity in the first postoperative day was acute in 6 patients (12 %), moderate in 10 (20 %), mild in 30 (60 %) and null in the other 4 (8 %). No patient has referred postoperative incontinence and stenosis has not been found in the stapling line during the follow-up. CONCLUSIONS. Our short-term experience has been satisfactory. It is a simple and safe technique with a short learning curve. The main postoperative complication, pain, had a behaviour similar to what has been published by other authors and it was easily controlled with non steroidal anti-inflammatories. However, a long-term follow-up is necessary to assess late complications, such as stenosis and recurrence of hemorrhoidal symptoms.
