A prospective economic study of cost-effectiveness with primary effectiveness data from a randomized clinical assay was carried out, in which two spinal anesthetic substances were compared: 5% lidocaine and 0,5% bupivacaine. The effectiveness was evaluated in terms of the probabilities of occurrence of transient neurological symptoms, which are an adverse reaction inherent to this group of drugs. Two groups of patients were randomly assigned, who required a less than 120 min. surgery under the navel, with the application of each studied anesthetic options to each group. One hundred and nine patients were administered lidocaine and 97 were applied bupivacaine. An expert in Neurology blindly evaluated the neurological condition of each patient before and 24 hours after the surgery. The costs of each option were estimated including those due to neurological toxicity. Cost-effectiveness ratio and incremental costs were also determined. Bupivacaine turned to be the option with a better cost-effectiveness ratio
