Resumen

BENITEZ LLANES, Orestes; GOMEZ BARRY, Hilario; CASTANER MORENO, Juan  y  FUENTES ABREU, Jorge. Elementos de predicción pronóstica en la nefropatía primaria por inmunoglobulina A. Rev cubana med [online]. 2003, vol.42, n.2, pp. 103-107. ISSN 1561-302X.

356 kidney biopsies performed to 346 patients from October 31, 1988, to October 31, 1999, were analyzed. It was proved that 66.5 % (230/346) had primary glomerulopathy and that IgA primary nephropathy (IgAPn) accounted for 7.2 % (25/346) of the total of biopsies performed and for 10.9 % (25/230) of primary glomerular diseases. The following variables were analyzed: age, sex, race, blood pressure readings, persistent proteinuria, histological pattern and kidney function. Average age was 23.8 ± 7.6. It was found that the white patients (18/25; 72.0 %) and the males (in a ratio of 2.6:1) were the most affected. At the end of the study, 20 % of the patients (5/25) with IgANp presented different degrees of renal failure, with a very significant statistical association (p < 0.01) when the kidney dysfunction was related to maintained arterial hypertension, a histological pattern of diffuse mesangial proliferative glomerulonephritis (DMPGN) and interstitial fibrosis in the histological study obtained by kidney biopsy. It was concluded that the mantained presence of AHT, a histological pattern of DMPGN and the finding of interstitial fibrosis represented a poor prognosis in the evolution of the disease.
