The response to the use of betamethasone as a fetal pulmonary maturant in 28 to 33 week pregnancies was studied. The universe of study was made up of 124 patients, 67 of whom were administered antenatal corticosteroid. The control group was composed by 57 patients with no treatment. Betamethasone was associated with the extension of the period of latency (over 24 hours in 79.1% of patients under treatment and 54.3% in untreated patients). According to the mode of delivery, the pre-term neonates born by means of cesarean section showed higher incidence of hyaline membrane disease (HMD) in both the treated group and the control group, compared with transpelvian deliveries where HMD was present in 2.6% and 6.9% of neonates of the treated group and the control group respectively. It was observed that Apgar score under 7 at 5 minutes was 4 times more frequent in the control group than in the group under treatment. In neonates from hypertensive mothers, the administration of antenatal betamethasone also reduces the occurrence of hyaline membrane disease. When we compare premature rupture of membranes with occurrence of maternal infection after delivery, we find that frequency of infection is 3 times higher than in intact membranes.
