A patient that was admitted due to proximal muscle weakness accompanied by increase of the mass volume of arms and legs is presented. Symptoms and signs suggestive of hypothyroidism that were later confirmed by hormonal measurements of T3, T4 and TSH, were detected. The presence of myopathy was proved by enzimatic and histic studies that allowed to confirm a hypertrophic hypothyroid myopathy or Hoffmann's syndrome. Muscle affectation was the form of presentation of hypothyroidism.
