A retrospective analysis was made in a sample of 15 patients operated on of aneurism of the anterior communicating artery by subfrontal interhemispherical at "Dr. Luis Díaz Soto" Higher Institute of Military Medicine, from January, 1990, to December, 1998. There was opening of the frontal sinus in 5 cases. The most frequent transoperative complications.were aneurysmatic rupture and lesion of the olphatory nerves in 3 patients each. The immnediate postoperative complications were. anosmia in 8 patients, hemiparesis in 2 patients and fistula of the cerebrospinal fluid in 1 case. Disorders of the conduct, memory or bradypsychia in 4 patients and sinusitis with mucocelle in 2 cases were among the late complications. 3 of the patients that underwent temporary clipping presented conduct disorders and 2 had hemiparesis. 3 patients with confirmed postoperative cerebral infarct had underwent temporary clipping and in one of them there was aneurismatic rupture. The outcome was not considered to be influenced by the surgical time. The conclusions of this analysis make us think that this is feasible approach from the surgical point of view, but with a percent of important complications that should be taken into account.
