A case of a patient with a spinal fracture at a level was presented of unstable L1 with neurologic involvement; grado C in the American Association of Spinal Injury scale was studied. The understanding of the vertebral channel was clinical and radiologically diagnosed and surgically treated in the first six hours of having suffered the accident, associated to the use of methylprednisolone. The patient in the immediate postoperatory presented a grade D in the ASSI's scale; 72 hours later had a grade E in such scale. It was proved the effectiveness of the medical treatment with methylprednisolona and surgical in the first six hours of trauma.
