Background: the diagnosis of sudden death infant syndrome, in relation to different definitions, classifications, complementary research and diversity of standards and protocols of performance, is difficult, so, a review on the subject is done. Objective: to update and to systematize knowledge on sudden infant death syndrome to contribute to disease´s diagnostic quality. Method: a bibliographical review of the syndrome was carried out in 250 bibliographic citations regarding causes, classification, differential diagnosis, etiopathogenesis, epidemiology, morphology, medicolegal aspects and others, by means of theoretical methods and documentary analysis, focusing on epidemiology and diagnostic aspects in 50 sources of information included in the article. Development: landmarks in the establishment of a definition and classification are related; some general and particular epidemiological aspects linked to the child, mother and the environment are presented. Morphological aspects from the accomplishment of a complete autopsy are approached, detailing external and internal aspects of the corpse, with histopathological studies and other complementary resources; including the research process on previous elements and around the disease. Aspects regarding its differential diagnosis are detailed, including its medicolegal importance according to statistical data and its form of presentation, it is essential to establish the type of death, its causes and the circumstances of occurrence. Conclusions: there is disparity of criteria and interpretations on epidemiological and morphological study of sudden infant death syndrome, that is why researching is so important and researchers should be scrupulous in reporting how they have made the research and how they have concluded it, in relation to the existing international definitions.
