Introduction:
Sepsis represents an indisputable and serious global health threat, with high mortality in intensive care units.
Objective:
To identify risk factors related to death in patients admitted to the intensive care unit with sepsis.
Material and Methods:
An observational, analytical and prospective study was conducted in patients admitted to the Intensive Care Unit at "Aleida Fernandez Chardiet" Teaching Clinical Surgical Hospital from 2018 to 2019. The study group consisted of a total of 224 patients.
Results:
In the bivariate analysis it was observed that patients with serum lactate˃ 2 mmol/L, PaO2/FiO2 ratio< 200, DA-aO2˃ 200, serum creatinine˃ 160 mmol/L, and total cholesterol levels< 3,9 mmol/L were at a higher risk of death (p< 0,001). According to the multivariate logistic regression model, the variables significantly associated with death were invasive mechanical ventilation (OR= 3,8; 95 % CI, 1,7-8,6; p= 0,001), serum lactate levels˃ 2 mmol/L (OR= 3,9; 95 % CI, 1,9-8,2; p˂ 0,001), PaO2/FiO2 ratio< 200 (OR= 2,6; 95 % CI, 1,2-2,7; p= 0,012), serum creatinine levels˃160 mmol/L (OR= 3,4; 95 % CI, 1,4-8,1: p= 0,005), age≥ 60 years (OR= 3,1; 95 % CI, 1,4-6,8; p= 0,003), and white blood cell count˃ 12x109 (OR= 2,2; 95 % CI, 1,1-4,5; p= 0,031). Hosmer-Lemeshow test indicated an excellent calibration of the model (p= 0,471) with an area under the curve value of 0,864 (95 % CI, 0,817-0,912, p< 0,001).
Conclusions:
In these patients, the factors related to death were invasive mechanical ventilation, serum lactate˃ 2 mmol/L, PaO2/FiO2 ratio< 200, serum creatinine˃ 160 mmol/L, age≥ 60 years, and white blood cell count˃ 12x109.
