Introduction.

Craniopharyngiomas are regarded as a complex molecular disease, with great neurovascular invasiveness and adherence in the skull base. Objective: to show an endoscopic model of surgical dissection based on new tumoral imagenological classification.

Methods.

A descriptive, longitudinal and prospective study was performed in 50 patients operated by Extended Endoscopic Endonasal Approach following the rule of 3 of surgical dissection: trianatomic complex exposition, sagittal trilaminar dissection, and axial vascular tripod dissection based on new imagenological classification.

Results and discussion.

Gross total resection was performed in 90 % of patients and 80 % was incorporated to normal live and work. Surgical complication was minimal, and insipidus diabetes was the most frequent. The disease-free survival during eight years was 81,6 %.

Conclusions.

Extended Endoscopic Endonasal Approach following the rule of three of surgical dissection represents a paradigm in the craniopharyngioma treatment.
