Introduction:

there are limitations in coordination between the primary and secondary levels of health, and there is a lack of a clinical history model for surgical activity in major outpatient and short-stay surgery, which affects the quality of this type of care modality.

Objective:

this paper aims to evaluate a proposal for a clinical history model for major outpatient surgery and short-stay surgery.

Methods:

a descriptive prospective investigation was carried out from September 3, 2012 to March 20, 2018. The clinical history model was evaluated by 90 experts with experience at the primary and secondary levels of health, who endorsed the quality and relevance of the proposal. Clients of the research project (from which it is derived) and institutional and provincial decision-makers also endorsed the proposal.

Results:

the evaluations carried out by the experts, clients and decision makers endorsed the quality, relevance and possibilities of implementation of the clinical history model.

Conclusion:

the proposed clinical history model offers security for its application.
