Resumen

Introduction: Non-cardiogenic pulmonary edema secondary to negative pressure is a uncommon complication occurring after the upper airway obstruction with a higher rate of presentation in children, of a infrequent way in adults and no referred for the patients presenting with morbid obesity according the review bibliography. Objective: to describe the perioperative behavior and course of a morbid obese patient with pulmonary edema from negative pressure. Development: Its etiopathogenesis is diverse but with a negative and excessive intrathoracic pressure provoked by spontaneous forcible inspiration in a patient with a closed glottis (laryngospasm), leading to liquid transudation from pulmonary capillaries within the interstitial-alveolar space accompanied by hemorrhage or not. The resultant pulmonary edema appears in a spectacular way in minutes after the airway obstruction or in a different one after some hours. This potentially severe clinical picture usually responds well to treatment with oxygen therapy, mechanical ventilation to positive pressure and diuretics. Conclusions: The suspected diagnosis to adapt the treatment is fundamental in further course of this type of patient.
