The clinical case of a 7-year-old female patient, who was admitted to the emergency room of the General Hospital of Latacunga, Ecuador, for presenting 72-hour abdominal pain is presented. On physical examination, clinical signs of acute appendicitis and peritonitis were found, the white blood cell count showed leukocytosis with neutrophilia, the ultrasound was not useful. With the diagnosis of acute appendicitis, emergency surgery was indicated, in which a torsion of the cecal appendix was found, with necrosis and perforation in its distal third, covered by the omentum and an abscess in the Douglas cul-de-sac performed an appendectomy, cleaning and drying of the cavity plus drain placement. The patient evolved satisfactorily and was discharged from the hospital on the seventh day.
