The achievements of the National Health System in Cuba and the decline in fertility have caused an accelerated aging process in recent decades, reaching at the end of 2020 the figure of 21.3% of the population aged 60 and over with an increase Of diseases. The development of new therapies and technologies for diagnosis and intervention in various areas of medicine make clinical trials essential, the participation of the elderly poses challenges for its design and development. The objective of this work is to make a brief description of the challenges imposed by the participation of older adults in clinical trials. For this, the information published in the PubMed database was consulted. An analysis of the difficulties that arise in the different stages of clinical trials was carried out and strategies were enunciated to improve the participation of older adults in these studies. It is concluded that it is very necessary to promote knowledge and compliance with the regulations established for clinical trials in populations older than 60 years, by researchers who design and participate in clinical evaluations of drugs and medical technologies.
