Introduction: the metabolic syndrome is a group of clinical and humoral alterations pathophysiologically related and sharing an atherosclerotic high risk. In this atherogenic process one of its first stages is the endothelial dysfunction. Objective: a randomized, controlled and clinical trial to determine the effectiveness of treatment with Metformin on the variables of the metabolic syndrome and the endothelial function. A group of 38 subjects presenting with metabolic syndrome was created prescribing a treatment with Metformin (1 700 mg.d) and another of 30 subjects also with metabolic syndrome treated with diet and exercises who were initially assessed and then after 3 months of treatment. Results: in Metformin group there was a significant decrease of the mean abdominal circumference (109.03-103.76 cm) of the mean blood pressure (98.03-92.98 mmHg) and of triglycerides (2.46-2.44 mmol/L). Flow vasodilatation increased from 2.21 to 5.94 % (p< 0.05). In group treated with diet and exercises there was an improvement of theses variables, but in the case of abdominal circumference and flow vasodilatation they were higher in the Metformin group. Conclusions: Metformin was useful in the control of the variables of metabolic syndrome and in the improvement of endothelial function of these patients.
