Herpes zoster is a viral disease provoked by the varicella-zoster virus, causing varicella. It takes place beginning from the reactivation of the virus, latent in the ganglia, provoking more or less serious skin damages, and mainly affecting immunocompromised individuals. We presented an interesting case with the objective of presenting the clinical features and the behaviour followed in a patient with herpes zoster. A female white patient, aged 51 years, assisting the consultation with pain in the right flank and lumbar region. Complementary exams were made as leukocyte count and electrocardiogram with negative results. Alter 24 hours, pain became more acute and irradiated to the right iliac fossa. A surgical procedure was decided due to a possible appendicitis; the surgical inform showed a little catarrhal. Pain continued and erythema and vesical lesions appeared in the right lumbar region that extended to the right flank. The dermatologist arrived to the conclusion that the diagnosis was herpes zoster. In a case of a patient with abdominal pain, the herpes zoster diagnosis should be taken into account for achieving a timely treatment and avoiding complications.
