Introduction: obesity is a chronic disease associated to physical, pharmacological and physiological changes which should be taken into account during the preanesthetic evaluation. Objective: update information about obesity and its implications from the perspective of anesthesiology. Method: non-systematic narrative review of the literature published in the last 20 years in the databases PubMed, Embase and LiLacs. Results: when dealing with patients scheduled for elective and emergency surgery, anesthesiologists are faced with one of the most common morbidities: overweight and obesity. They should be trained to avoid complications related to the increase in fatty tissue, such as changes in the patient's airways and in their cardiovascular, respiratory, gastrointestinal and nervous systems. They should know about the pharmacokinetics and pharmacodynamics of the metabolism of these patients, and anticipate postoperative situations requiring intensive treatment. One of the treatments proposed for the management of morbidly obese patients is bariatric surgery, an additional challenge for anesthesiologists. Conclusions: success in achieving high survival rates will surely depend on the work of anesthesiologists.
